There are several options for scheduling an appointment.

For New Patients, please call 866-228-1108 or complete online referral form or have your physician send in a referral.

Current patients (if you have been seen within the last three years), please complete online form or contact your clinic directly.

A copy of your insurance card, Valid ID, a list of any medications, and any scans/X-Rays or notes. If you are having a procedure please make sure you have transportation.

Yes, SEPSC will file your primary and/or secondary insurance to verify what portion of your visit they will cover. Any remaining balance will be billed to the patient.

The reason that you are receiving two bills is because Southeast Anesthesiology Consultants is a separate company contracted by your hospital to provide you with pain services.
